WHO CAN NOMINATE:
Nominations may be made by any individual (alumnus/alumna, Lasallian partner, employer, colleague, friend, former classmate or teacher, or enrolled student at DLSL) or alumni batch/class. A nominator may submit no more than three (3) nominees every award year. Members of the Gawad Alumni selection committee are not eligible to make a nomination.
Alumni previously nominated for the award may be nominated again by qualified nominators. This should be done by attaching up-to-date information to the nomination form.
Prior to submission, nomination must be duly accepted by nominee.
NOMINATION REQUIREMENTS:
The nominator must submit the following in order to consider the nomination:
• Nominees must be graduates of DLSL Elementary, High School or College (including MMT) or OLRA (1963 graduates only).
• A filled-out nomination form. One form for each nomination
• A letter supporting the nominee stating the reason/s for the nomination in a particular award category. The testimony must not be more than two pages in length.
• Three (3) written, dated, and signed testimonials in support of the nomination coming from individuals not related to the Nominee either in consanguinity or affinity.
• Executive summary for updates on career, membership in organization, other important information to support nominee's exceptional achievements. (See sample Data Sheet Below)
All requirements must be mailed in a sealed envelope on or before April 30, 2018 to:
